Purpose:

To deliver a trusted assessor service to support the discharge of residents who have Adults Social Care input back to where they were living prior to going into hospital.

 

Decision:

To approve the Trusted Assessor service for 2 years supporting with hospital discharges across all hospital bases including community hospitals and hub beds.

 

The contract value is £125,000.00 per year for 2 years (Total Value £250,000.00).

 

Reasons for the decision:

The ‘Trusted Approach’ approach is one of the 8 High Impact Changes set out in Better Care Fund (BCF) planning guidance and endorsed in the Hospital Discharge Policy requirements published by NHS England (2022). The underlying principle of the approach is to promote safe and timely discharges from NHS Trusts to adult social care services.  The Trusted Assessor approach continues to be part of BCF planning guidance which was issued on 22 January 2025.

 

Alternative options considered:

This is a direct award due to the experience needed to provide this type of service.

 

Exemption for the direct award has been approved.
